Can insulayment be doubled up under ceramic tile?

No, this would void the warranty and can cause issue with the floor if you double up any underlayment.

How thick should the thinset be (1) under and (2) over the underlayment ?

For thinset, I would use as per thinset specifications. We have it directly underneath the hardwood floor, between the hardwood and subfloor. Works excellent. In the thinset or tile parts of house, this has little to no sound improvement. I definitely would not use this in thinset/tile again.

It should. You'd probably have to trim it so it was able to lay flat on the tread. I don't see any benefit to covering the nosing or the riser.
