Could you use the cooking grate upside-down so you have a lip to keep food from falling/rolling off?

yes. it just slips on over the pole so you could turn it either way. good idea actually!

Does anyone know if this actually swivels? And does it make the support in the ground loose if it does?

Yes it does swivel and no it does not make the support in the ground loose. This is my second summer using this grill and I love it.

How low can you clamp the grill?

The grill has a easy clamp. You can put it as high or low as needed.

Will this hold up at the beach / in sand?

Yes. It should work great on the beach. The end that gets pounded into the ground has a wedge to keep it secure.
