Does it work with google fi data only sim card?

UPDATE: It no longer works with Google Fi. Google decided to stop allowing Wifi Hotspots.

Can it use any sim card or only a providers sim card?

Unlocked devices are compatible with GSM carriers the kinds that Use SIM Cards for Service like AT&T and T-Mobile as well as with GSM SIM cards (select prepaid carriers). Unlocked Devices wont necessarily support CDMA Carriers the kinds that don't use sim cards for service like Sprint, Verizon, Boost or Virgin, it's always better to contact your service provider prior to purchasing to confirm their compatibility on the bands supported by this device.

Does this require a monthly fee/subscription? Can you use it even where wifi is not available?

I used ATT pre-paid and T-Mobile postpaid SIM cards and both worked in my unlocked version.

Does it use a 5ghz radio frequency?

Yes, it supports both 2.4 GHz and 5 GHz connections (all 11ac devices support both bands).
