Can you listen water leaks under cement?

I don't think this instrument is much more "sensitive" than if you were to put your ear on the cement, and listen. You might want to consider getting an ultrasonic listening device for that application. Just my take.....

Can this help pinpointing rattles in the car?

This device has a solid metal thin probe. If you touch a something suspected of being the source of the rattle with the probe, it essentially amplifies the sound of what it is touching, and you hear that sound. The key point is that it is made to physically touch things. My personal approach to finding rattles in a car (not that how I do things matters) is to have someone else drive the car, and you be a passenger, so you are free to move about within the car, using your ears and hands, and listen and feel from where it's coming from, and locate it that way. This device is more for touching under-hood components like the stationary parts of items that have spinning pulleys, to hear if the bearings are making noise.

Will this locate a bad rocker arm in a penastar 3.6 engine?

This is an acoustic device. That means, it "hears" noise - or you do, because it amplifies the noise for you to listen to. IF your rocker arm is making noise because it is bad, that is - it is making more noise than the ones next to it - then you should be able to hear it.
