Can these be worn over hi-top steel toe work shoes and if so how do I go about measuring ?

No these are like shoes, they are better than rubber pull over. I had a pair of the pullover, they only lasted a month, no tread on them

Do you have size 6(USA)?

Dear customer, you can refer to our size.

If he's a men's US 10.5 what size should he order?

There is 10.5 size in the 'size list' tab. It should work. I have small feet so I wouldn't know for sure though.
