No it dose not smell, and there is a washing guild.

I turn a lot during the night, does it stay in place?

If you place it correctly between your knees it should stay in place. I use mine just above the knees between thighs and it stays there even though I change sleeping sides frequently.

Can you remove the cover for washing, it seems it would be very difficult to get back into the cover. Or, can be buy loose cover?

Yes you can wash the cover. It's a bit fiddly to get it back on but if you squash the foam you can do it. I would hand wash the cover.
