Does this have to be used upright or could I use it horizontally to steam items such as carpet?

I don’t know that it’s intended for carpet, and you’d probably have to refill it constantly, but I don’t see an issue with using it horizontally since it isn’t supposed to leak. My only concern would be the water in the reservoir shifting off the heating element when tilted, resulting in a lack of steam.

How hot does the steam get? Can it sanitize?

it gets quite hot you have to be careful as it can burn you if you get you hand in front of it. im not sure if it can sanitize but it probably can.
