Hi, does this brief sit under the bust and have an opening for using the toilet

Yes to both although I didn't use the toilet opening

Do these give you a thigh gap?

The product is very good but I am not sure about the thigh gap as this may depend on weight.

Is the waist size guide in inches?

It is but it's the hip measurement shown not the waist measurement.
