Is this wide enough for a floor length flexible dress and is it regulation carry-on size?

A medium mans dress shirt can touch the bottom of the available space, a dress would need to be folded

can it hold a size 46 suit?

That’s my size, I found that the bag is just a bit short where at the bottom of your suit may be a bit bunched up. But if you can live with it, my suit wasn’t wrinkly, it does what it says it does. I love and looking to purchase a a few for Christmas presents.

Will this fit a 54r suit jacket?

Yes, along with a few shirts. You may have to fold the arms in. My husband loves it.
