My friend has ibs and after his stroke he sometimes can't hold his bowels. Can these handle that kinda mess?

Hello Christina. Our Depend Real Fit Briefs for Men are designed for urinary incontinence, so we wouldn't recommend it. However, our Depend Adjustable Underwear are made for both urinary and bowel incontinence. You can see them here: https://www.amazon.com/Depend-Adjustable-Incontinence-Absorbency-Disposable/dp/B00G7VO5S4/ref=sr_1_1_a_it?ie=UTF8&qid=1519654386&sr=8-1&keywords=depend%2Badjustable%2Bunderwear%2Blarge%2Bx-large&th=1. Thanks and hope this helps!

Wi have a 40 inch waist. should i order s/m or l/xl?

My husband's waist is 44 and these are just right for him. I would try the l/xl then downsize if needed. They expand as needed but aren't baggy.

I use at least 4 to 5 depends guards a day. how does this compare to depend realfit?

Hi there, our Guards hold between 8-10oz and our RealFit holds between 18-22oz. We hops this helps!

When are they going to make a size big enough for a 48-50 inch waist (i.e. a big butt!)? these "xl" are not "xl" enough.

As a wife with a man with a large tummy, I agree! They should have four sizes~~Small, Medium, Large, and Extra Large. I would send the manufacturer a complaint if I were you.
