It's potential to wrap tightly around the knee and provide "cold" behind the knee were the selling points.Instructions are very clear, and fitting whilst seated is the better position as standing brings in gravity.Remove from freezer compartment of fridge where it fits snugly in the ice maker draw. Remove from bag, if it's been in the freezer for a long period allow it to thaw slightly as the gel is less flexible.Wrap around the long straps first and tighten, the Y-yoke second. Raise leg onto support and allow the magic of ice to work. On warm days the cold effect lasts about 20 minutes, which for me is perfect. It is reversible. A fabric side allows safe contact next to the skin and non-material side allows better "cold" effect if wearing anti-thrombosis stockings or similar. The straps are velcro'd, well stitched and long enough to wrap around the largest knee.
