No safety pins or wearing things over my neck. Sleeping with it is easy, I just shift the top over, so I'm not sleeping on the external stuff. Also has 2 pockets like a hoody on the outside, so handy for my phone. .I highly recommend getting at least two, so you have one to wear while washing the other. It's nice enough to wear out and comfortable to sleep in. I went up a size for the bulkiness of the vacuum pump, and glad I did, gave me more room to shift it over so I'm not sleeping on anything. Having the zipper is much easier than buttons. This is a game changer making recovery so much easier!
