It vibrates for say 3 seconds and then its off for 1 or 2. Then back on. There are 3 of these settings, setting one my guess is 1 second on 1 second off. The vibration is strong enough that you can feel it. I wouldn't think it would help with the pain but it does. The heat is best but the vibration just helps loosen it all up. If I had to come up with a complaint it would be that some days I wish the belt had twice or even three times the coverage because my back is so bad in many places I find myself using this on my lower back, then I have to move this to my middle back and then move it again to my upper back.
