His hand and arm are especially cold so I thought since this is 26" long we could warm it in the microwave and wrap his hand & arm. It works perfectly and retains the heat for quite a while before needing to be re-heated. Also early dementia, which makes her sensitive to cold especially. Heating this up for her makes her so happy. I drape it around her neck and she says, "oh, that's so nice" every time. While it's heavy, when it's on, the weight is distributed evenly and it's comfortable. Don't over heat it - it can get a bit too hot. But if you do, just let it sit open for a few minutes to cool a little. I find 2.5 minutes in the micro is just right. Mom often falls asleep with it. She's used it on her knees, too. Anything to help her is great, and she really enjoys this item. She also loves purple, so that worked out well! It does have a very slight herbal scent to it - very slight. Mom is sensitive to odors and will mention when something bothers her, but this does not, so I wouldn't worry about the smell. It did dissipate quickly after the first few uses but you can still notice it a little. Nothing I'd be concerned with.
