If you plug this into your ears and hold it on someone's heart you can hear something but it's not very clear. We bought it because our Shepherd is very anxious about the vet's and using this on him before he goes, helps to calm him down. **Q: Would this be too big for a 20" doll?** A: Yes. **Q: What items go with this? Most doctor play sets are for very young children.
