I have tested this without a layer of clothing for protection and it was as comfortable as any gait belt of this design can get. It's a solid quality belt, one that my physical and occupational therapists both commented on. It also has a belt loop so the end doesn't flap around. The fabric is heavy grade canvas, nice and sturdy, which is what you need for this application. There aren't any instructions on how to buckle it correctly so that it doesn't slip loose, so ask your therapist how to do it (or there's always YouTube!). I leave mine buckled and just slip it on and or over my head.
