Its a long story - but don't ever allow anyone in the dental profession to override your wishes and give you injections twice in one week, as well as prolongedly having your mouth open twice in one week due to his own error of misjudgment!) Once i adjusted the velcro to fit me, I simply keep it that way, and alternatingly insert the little ice-packets as needed. I was also very relieved that there's no odor, as I'm chemical sensitive One thing I should add, those round little "bonus" packs that were thrown in were worthless to me.
