I initially tried this without the sheet, but discovered that I needed to move more slowly and was able to remain on the mat much longer the second time. I wasn't sure what exactly to expect. The box refers to a bed of nails and the health that is increased. The mat and pillow have little round plastic pieces with points that are meant to stimulate your pressure points (acupressure). It's a good quality and comes with a nice carry bag, should you want to travel with it. They suggest lying on it for 10 minutes to start and say you can use a sheet. I did need to use a sheet - the first few minutes were too intense. I felt my heart rate rising. After a week or so, I can now do a full 10 minutes without the sheet.
