A: Small would fit better.  I got medium for husband who it s 38-40 chest.

A: Not sure but I bought two XXL which was fine in the Regatta Thompson fleece (approx 48" chest)....these were very small ( I'd guess approx 44-46) in size and returned...I'd say go up one or two sizes from your usual.

For the price I paid I would not expect anything more. This fleece is light weight, warm and packs away small. It might not be the thickets or warmed fleece ever but for an extra layer it is perfect for throwing into a backpack when hiking or camping due to its compact size and weight. It also fits really well.
