We have a Chamberlain LiftMaster opener. I believe it is a 2004 model based on the year our house was built. I did not look up the specific model number, just the brand really.I tried following the directions on the website/paper slip included in the packaging however none of it worked. I ended up following the directions directly on my garage opener and that did the trick. Took me less than 5 minutes to do, super easy and SO much cheaper than the ones at Home Depot. The only thing is (and this is not a defect but just a tad annoying) is that when you click the button to open or close it, you have to press and hold the button for 1-2 seconds for it to work.
